Request evaluation license key form

Crate a form with on dedicated subpage with the following fileds:

• company name
• email
• Company VAT ID or registration number
• country
• address (street, town, postal code)
• choice: license period 7 days; 14 days; 28 days.
• add option to mark licenses as evaluation, so its easier to track which license was issued for evaluation purposes

So that a prospect can request an evaluation license.

Request should be stored in dedicated google spreadsheet.

There must be a way to approve the request.

After the request is approved a request to license server must be made to create a client and a license (default 14 days).

The license should be then automatically sent to the prospect email address.

Restrictions

• only one evaluation license can be created for a company - based on VAT ID?

Do debugowania : czasem formularz nie wysyła requestów do serwera. Ja miałem jeden przypadek i jeden z klientów też (wypełniał przy mnie). @jakub-tldr zrób proszę na to testy.
