test: add comprehensive tests for structured output functionality

- Add unit tests for new data structures, `WithOutput*` functions,
  and helper functions in the `mcp` package.
- Add tests for the schema validation logic, covering both valid and
  invalid structured content.
- Add integration tests for the server to ensure the validation step is
  correctly triggered and handled within the tool call lifecycle.
