Custom Chat Mode shows "Agent Mode" in welcome message

Does this issue occur when all extensions are disabled?: Yes/No

• VS Code Version: 1.102.0
• OS Version: MacOS/Windows

When creating a custom chat mode, the "Welcome Message" shows that you are in "Agent Mode" as seen in the screen shot where I have "Plan" selected which is custom. However, the three built-in modes show a different message for each. It would be less confusing if it showed the name of the custom mode instead of "Agent Mode" and used your description in the main message, or had another field within the .md file that would display in that area.

Thanks for creating this issue! It looks like you may be using an old version of VS Code, the latest stable release is 1.102.1. Please try upgrading to the latest version and checking whether this issue remains.
