What steps can reproduce the bug?

1. Toggle /approval and set to read with request for approval
2. Ask codex to implement change, it fails to edit
3. Mention clearly: "YOU CAN ASK FOR EDIT APPROVAL REQUEST so this environemnet is not just readonly !!"
4. It keeps failing

What is the expected behavior?

Codex should ask for edit approval and actually edit the file.

What do you see instead?

Codex doesn't edit the file.

I looked into this and found that the "Don't Trust" option wasn't applying the approval policy correctly. Opened a PR to fix it: #4941
