Change separator for conditions of different clinvar variants

The conditions of a clinvar variant ID are separated from those of an other ID with , , which is not easy to interpret because some condition names include commas themselves (e.g. Breast-ovarian cancer, familial, susceptibility to, 3).

Fix:

• Change separator from , to | to better distinguish the conditions of the different clinvar IDs.
