Testing: No coverage for "should fail" cases

There are no tests for error handling or "should fail" cases, and apparently no way in the test frame to add them.

Example Gaps:
  - No tests for detecting file format or encoding errors
  - No crash tests for the error handling subsystem
  - No tests for consistent/correct error messages
  - No tests for error condition handling
