This PR is part of an effort to bring the cuDecomp repository in line with current NVIDIA OSS policy.

In particular, this PR relicenses cuDecomp from the current BSD 3-clause license to Apache 2.0. The reason for this change is that NVIDIA policy requires external contributors to sign a CLA, which is a bit heavy handed for this project. For Apache 2.0 licensed projects, contributors are only required to sign-off their commits with the Developer Certificate of Origin (DCO), which is a simpler process. The information about DCO sign-off is included in a new CONTRIBUTING.md file.

We have a couple of external contributors so I will request confirmation that it is ok to relicense your contributions before merging this PR.
