Description of bug:
The NMAHPs survey form jumps straight to the questions. The demographics survey is not showing at all. I tried two versions and both have the same glitch.
How can we reproduce it:
Go to the survey & you can see it. the nurses survey did not have this issue.
Expected behaviour:
The demographics question section should follow the consent page.
Device I'm using: Windows PC
