Debug mode for fhirpath expression evaluation

For complex expressions it would be helpful to have an instrument to introspect engine state and execution status.

This could be as simple as environment variable toggle that switches debug logging on.

The fhirpath lab now has this functionality for the HAPI engine, and is working towards getting other engines to also support the same interface.
