Enable the entry of schema changes in the class creation form

Describe the solution you'd like
Add a text box to capture information on schema changes. Add the property dct:provenance to the created class and save the captured information as the value of this property

This information is currently being captured in the description of the class, but it would be better to capture it elsewhere.
