Get Windows CI working (probably appveyor.com)

Get a Windows CI server working.  Anything!

appveyor.com looks good.  It provides MinGW, VS2013 and VS2015 environments.  I've already added piledriver (see https://ci.appveyor.com/project/schwern/piledriver) and am working on its build configuration.
