Ego-Exo4D license request form shows “Failed to fetch” on submit

I’m trying to submit the Ego-Exo4D license request form, but the submission fails with the message “Failed to fetch” right after I click Submit.

Getting 'template ids must be array of string' dialog window when clicked submit to sign license ... did anyone face this?

Hello, we are working on rolling out a new license request interface. We aim to launch this by end of Feb. Hopefully that will resolve these persistent issues. We really appreciate your patience.
