[Enhancement/Feature Request]: Add Phone Number field under user creation #1553

Use case/Problem Statement
Add phone number field under the user creation window.
- We have been previously adding the phone number under the email field but this was updated to validate the email format.
If present the phone number should be added as an addional entry to the Practioner.address field using the ContactPoint data type with the system value as phone
Acceptance criteria
- Phone number field added to the creat user window
- Phone number saved as an additional entry to the Practioner.address field
