doc update

I've added "intercolumnar" to @place on fw, add, addSpan to support Junius 1, in consultation with @zabt and @mmwwah. Could the doc be updated to match?

Also, I notice on note and ref that it doesn't choose between anchor and ptr. Pragmatically, it should; ptr would be better.

Good one! I want to move the Guidelines over to GitHub, as they will be easier to maintain that way. I'll get onto this as soon as I get back from Kalamazoo.
