fix: flatten wiki pages to root level for GitHub wiki compatibility GitHub wiki doesn't support subdirectory paths in URLs. Pages stored in agents/llm-agent.md must be accessed as wiki/llm-agent, not wiki/agents/llm-agent. Copied all subdirectory pages to root level and fixed Home.md links to use flat page names.
