perf: master agent default prompt & plan agent output json schema

• plan agent 输出为 json 格式, 方便前端去做解析渲染
• 默认禁止 plan 调用 tool_choice, 但是 tools 数组正常传递
• claude 模型在传 tool 时需要加一个空的 parameters, 否则调用会报错
