Additional Feedback Form Requirements

Problem

After meeting with Sunil and Louisa, some additional requirements for the form had spawned

Solution

The following has been added:

• Organization unit and playwright tests
• Moved the FeedbackCTA above the name in mobile.
• Removed phone numbers from the displayed list of issues.
• Updated the Submit button so it is no longer disabled; users can now select it and see validation error messages instead.
• Increased the character limit in the details box to 750 characters.
• Updated the success message title and subtitle copy to match the Figma.
• Added the “Report another issue” button.

Result

The feedback form is more stable and has a better user experience.

Test Plan

• run make up
• go to either practitioner or organization
• go through the feedback flow
• visit /admin/ and view your submission
