docs: round-3 polish + LEARNINGS; bump to 1.4.1 (migration-ready) - #28

Round 3 of the blind cold-clone audit found no material repo-fixable gaps (converged). This closes its minor findings and wraps the exercise: - CLAUDE.md: the issue-vs-PR numbering note claimed 'different sequences' — GitHub issues and PRs share one sequence (the reader proved it from our own disjoint numbers); corrected. - Docstring + schema.md + README: state explicitly that tasks do not inherit their parent feature's @milestone= (rollup counts task tags only; --table attributes features directly or via tagged tasks). - CONTRIBUTING.md: add the squash-merge/never-main rule and the gh-release step so the public doc matches actual process. - LEARNINGS.md: entry for the blind-dogfood migration exercise. - MEMORY.md: self-healing tag note (v1.4.1 committed; tag pending owner confirmation) + audit trail in 'what just happened'. - __version__ 1.4.1 + CHANGELOG entry (docs-only release, keeps the version/tag-consistency invariant when the migration tag is pushed). Round 3 blind reader (fresh agent, blind to rounds 1–2) rated every finding MATERIAL vs MINOR: zero material repo-fixable gaps — the loop has converged. The one MATERIAL item it kept is inherent and correctly mitigated: the roadmap's source of truth is the GitHub tracker, and the reader's own first action was to re-verify #14 there before coding, which is exactly what MEMORY.md tells it to do.
