This PR adds a table of contents to all three markdown files.
The TOC was generated using https://github.com/jonschlinkert/markdown-toc.
If there are any tweaks needed, I'd be happy to do those.
I also thought to maybe include some extra info per section-header, like a small link back to the TOC or back to its parent. That could help navigating. This idea is not yet implemented.
