Adds a skill for creating feature request Stories in the CI Jira project, matching the structure of Dimagi's form 134. - Collects summary, user story, priority, current/expected behavior, impact, CCC program, CCC product area, and components - Shows a formatted preview before creating - Auto-applies the `form` and `form-134` labels on every ticket - Populates CCC custom fields (customfield_10764, 10765, 10766) - Defaults priority to P4
