What does this PR do?

to further refine our issue backlog, create a template for maintainers/contributors/users to open issues
related to our documentation

Other issue templates do, but if the maintainers prefer I'm happy to remove them 😄

I don't think this is necessary really. What's the evidence people are finding it hard to do this? Increase in boilerplate like this does no service to the project.

@ashwinb I've been away on PTO - can you explain your opposition? Right now we have no way to filter documentation issues/template for folks to report these issues - I've seen issues opened as Bugs or Tech Debt which IMO makes the backlog of issues much harder to wrangle for folks who are interested in working on docs issues.

It also would be nice if we could leave these open to allow authors to respond instead of just closing - I have several PRs that have been waiting for review for several weeks with no response yet this one was closed presumably as part of some PR grooming that has occurred?
