Proposed change(s)

Right now we have two separate package com.unity.ml-agents and com.unity.ml-agents.extensions in one repo.

We want to move com.unity.ml-agents.extensions into com.unity.ml-agents for MLA 4.0.0. Make sure the corresponding docs are updated as well.
