Update vulnerable dependencies

Updates HAPI FHIR to 8.12.1 and SnakeYAML to 2.7, and constrains XStream, BeanUtils and JSON Smart to fixed versions. Includes the API and test adjustments needed for these updates, with missing CDC terminology bundled for local US Core validation. Java requirements are unchanged.

Validation:

• Java 17: check shadowJar passed; 643 tests passed, 6 existing skips.
• Java 25: assemble passed.
• Trivy: HIGH/CRITICAL findings in the bundled JAR dropped from 83 to 0, without an ignore list.
• Three seeded patients produced matching FHIR, CSV and CCDA output, apart from the FHIR build-version text.
